HEALTH REQUIREMENTS
The New York State Education Department requires a physical exam for all students in grades Pre-K or K,1, 3, 5, 7, 9 and 11 and all students new to the district. New York State Law also requires routine health screenings (height, weight, vision and hearing) on each physical exam year.
Effective 2/1/21, all health examinations performed for school must be documented on the NYS Required Health Examination Form - pursuant to Education Law. Health exams must be performed by a NYS medical provider. Schools cannot accept the health exam if it is not on the required form. See the attached copy of the required form, listed under FORMS below, to bring with you to your Health Care Provider (HCP).*
A physical exam completed by your private physician is valid for 12 months. The exam must be completed within 12 months of entrance to the required grade level to be valid. Please bring physical exam form to your child’s doctor to complete. These exams are due in the nurse’s office by October 1st.

CAN MY CHILD GO TO SCHOOL TODAY?

Staying home when sick can lower the risk of spreading infectious diseases like COVID 19 and the flu to other people. Children who have symptoms of respiratory or gastrointestinal infections like cough, fever, sore throat, vomiting or diarrhea, should stay home. Your child shouldn't return to school until fever free for 24 hours without the use of fever reducing medication, hasn't vomited in over 24 hours, and until generally feeling better.
